Rachel Lewandowski, APNP-CNP, an SSM Health St. Agnes Hospital Endocrinology nurse practitioner, is being recognized as an SSM Health OAK (One of a Kind) Award winner.

The OAK Award is specifically designed to recognize physicians and APPs from the Fond du Lac Regional Clinic, Ripon Community Hospital, St. Agnes Hospital, and Waupun Memorial Hospital who demonstrate great teamwork and communication skills, collaboration, professionalism, exemplary service, and a commitment to excellence.
